Hope Lives! Golfing for Hope – Waiver & Release of Liability

Assumption of Risk

I understand that participating in the Hope Lives! Golfing for Hope Golf Tournament involves inherent risks of injury or property damage. These risks include, but are not limited to, being struck by golf balls, golf clubs, or golf carts; slips, trips, and falls; uneven terrain; weather conditions (including heat, wind, rain, lightning, and humidity); collisions involving golf carts or other participants; actions of other golfers, spectators, or volunteers; wildlife; and other unforeseen hazards.

I voluntarily assume all risks associated with my participation in this event, whether occurring before, during, or after tournament activities.

Release of Liability

Having read this waiver and knowing these facts, and in consideration of the acceptance of my entry, I hereby, for myself, my heirs, executors, administrators, and assigns, waive, release, and discharge Hope Lives! (the "Organizer"), the host golf course, its owners and employees, tournament officials, volunteers, sponsors, vendors, suppliers, agents, independent contractors, employees, and any other affiliated parties (collectively, the "Released Parties") from any and all liability, claims, demands, damages, losses, or causes of action arising from or related to my participation in the Golfing for Hope Golf Tournament, including those resulting from the negligence of any Released Party, to the fullest extent permitted by law.

Indemnification

I agree to indemnify, defend, and hold harmless the Released Parties from any claims, damages, liabilities, losses, costs, or expenses (including reasonable attorney's fees) arising from my participation in the tournament or from my actions during the event, including claims brought by third parties.

Golf Cart Responsibility

If I operate or ride in a golf cart during the tournament, I agree to do so in a safe and responsible manner and to comply with all golf course rules regarding cart operation. I understand that I am responsible for any damage I cause to the golf cart or golf course property and assume all risks associated with golf cart use.

Photo & Media Permission

I grant Hope Lives!, its sponsors, and its authorized representatives permission to photograph, videotape, record, and use my image, likeness, voice, and name for promotional, educational, fundraising, and marketing purposes in any media, without compensation or further approval.

Representations & Agreements

I certify that I am physically able to participate in the Golfing for Hope Golf Tournament and understand the physical demands associated with playing golf and participating in tournament activities.

I agree to:

• Follow all tournament rules and golf course policies.
• Conduct myself in a safe and sportsmanlike manner.
• Follow the directions of tournament officials, volunteers, and golf course staff.
• Refrain from any behavior that may endanger myself or others.

I understand that the Organizer is not responsible for lost, stolen, or damaged personal property.

I understand that emergency medical services may not be immediately available and that I am responsible for any medical expenses incurred as a result of my participation.

Minors

Participants under 18 years of age may participate only with the consent of a parent or legal guardian. By signing this waiver, the parent or legal guardian agrees to all terms of this agreement on behalf of the minor participant and assumes responsibility for the minor's participation and conduct.

Acknowledgment

By checking the box below (or by signing a printed copy), I acknowledge that I have carefully read this Waiver & Release of Liability, fully understand its contents, understand that I am giving up certain legal rights by agreeing to it, and voluntarily agree to be bound by its terms.

Participants under 18 years of age must have this waiver signed by a parent or legal guardian. A signed paper copy may be required at tournament check-in.
